1. Take 680 South.

2. Exit at northern (first) Mission Boulevard Exit.

3. Turn right at Mission.

4. Turn left at Stevenson Boulevard.

5. Turn right onto Civic Center Drive.

6. Turn right into the Fremont Surgery Center parking lot. The entrance is located on the parking lot side of the building under the canopy.

From San Jose (Interstate 680):

1. Take 680 North.
