A Cost-Effective Healthcare Alternative
Employers, insurers and third party administrators understand that ambulatory surgical centers like Fremont Surgery Center are a high-quality, cost-effective alternative to having surgery in a hospital. Because Fremont Surgery Center's overhead is a fraction of the cost of a hospital, the savings are passed on to the consumer. A flat fee, charged for each procedure, covers the facility, customary medical supplies and nursing care.

Your Bill
We accept payment from most insurance plans. Our staff may contact you about your insurance coverage prior to your procedure. If you have any question about your bill, including coverage and pre-approval or other requirements, please contact your insurance company or our business office at (510) 793-4987.

As a courtesy, we will file your insurance claim for you. We will ask you to sign an "assignment of benefits form" so that payment from your insurance company comes directly to Fremont Surgery Center. Any amount not paid by your insurance, such as deductibles and co-insurance, will be collected on the day of surgery unless you make prior arrangements with us.

Our bill does not include any services from your doctor, the anesthesiologist, or any laboratory or pathology services. You will receive separate bills from them.

We will collect all fees not covered by insurance prior to your surgery. For your convenience we accept cash, cashier's checks, money orders, Visa and MasterCard payments.

Ownership Disclosure

The physician who referred you to Fremont Surgery Center may have a "significant beneficial interest" in this surgery center. If you have any concerns please discuss them with your physician.
